it by the school office, or mail it to St. Mary School, 306 3rd. St. N., Humboldt, IA 50548.

Attached below is an informational letter regarding the free and reduced lunch program, an
attachment on how to apply, and the application itself.

If your household received a letter from DHS called "Direct Certification Approval for Benefits",
you DO NOT NEED TO COMPLETE an Iowa Application for Free and Reduced Price School
Meals. This letter was sent out to inform you that your child has already qualified for free or
reduce priced meals. If the notice was received from DHS, please SIGN it and return it to St.
Mary School, or mail it to St. Mary School, 306 3rd St. N., Humboldt, IA 50548.
